The UnitedHealthcare Community Plan Long-Term Care (LTC) plan specialists can answer questions about this plan. 

To become an LTC member, the following steps must be completed prior to selecting an LTC Plan:

  1. Screening
  2. Eligibility
  3. Enrollment

Eligibility for enrollment in the UnitedHealthcare Community Plan Long-Term Care (LTC) plan is determined by CARES (Comprehensive Assessment and Review of Long-Term Care Services).
