We offer no or low-cost plans to help New Jerseyans get healthy — and stay healthy since 1989. We cover adults, people with disabilities, expecting moms and children with our New Jersey Medicaid and NJ FamilyCare plans. We offer a Dual Special Needs Plans for those who qualify for both New Jersey Medicaid (FamilyCare) and Medicare.
We’re proud that our NJ Medicaid (FamilyCare) plan is accredited by the National Committee for Quality Assurance (NCQA). NCQA is a private, non-profit organization dedicated to improving health care quality. UnitedHealthcare Community Plan of NJ has also achieved NCQA’s Long-Term Services and Supports Distinction for our UnitedHealthcare Dual Complete® plan.

NJ FamilyCare
Our NJ FamilyCare plan is for children, parent or caretaker relatives, and adults without dependent children. It's also for people who need extra support and care to live safely at home. You can choose your own doctor. Choose "View plan details" to see what our plan offers.

Individual & Family ACA plans
UnitedHealthcare offers Individual & Family ACA plans in New Jersey. They’re designed to give you the coverage you need without breaking the bank.
Choose from plans in the Bronze, Silver and Gold metal levels. They each come with a variety of benefits, including reliable support from our customer service experts.

Eligibility
New Jersey FamilyCare covers people including:
NJ FamilyCare covers low income children under the age of 18, pregnant women, parents/caretaker relatives, single adults and adults (without dependent children) ages 19-24.
Eligibility is based on family size and income. Family size includes children under 21, adoptive and natural parents, or caretaker relatives and their spouses.

MLTSS Program
The Managed Long Term Services and Supports (MLTSS) program is focused on serving frail, elderly and disabled individuals. This program combines traditional health care benefits (doctor visits, hospital stays, labs) and long-term services and supports (providing help in home or community with daily living activities) that includes:
- Care management
- Home and vehicle modifications
- Home meal delivery
- Mental health and addiction services cover counseling and other treatments
- 24/7 personal emergency response device to get you help quickly at no cost
- Personal care

Healthy First Steps®
The Healthy First Steps team is here for you. Our program will help you take the right steps to keep you and your baby healthy.
We’ll help you:
- Choose a pregnancy provider and a pediatrician (child’s doctor)
- Schedule visits and exams and arrange rides to your visits
- Get supplies, including breast pumps for nursing moms
- Connect with community resources such as Women, Infants and Children (WIC) services
Health education
UnitedHealthcare Community Plan of New Jersey offers free health education to help everyone live healthier lives. We offer programs for children, parents and seniors, and that cover men and women’s health issues.
Topics include:
• Asthma • Lead poisoning • Childhood obesity • Diabetes • Heart disease • Injury prevention • Medication safety • Nutrition • Make your doctor’s visit worthwhile • Parenting skills • Stress management • Understanding breast cancer

Interpreter services
You and your providers need to understand each other. Not speaking English makes this difficult.
We can arrange for an interpreter to be at your appointments.

Community Connector
Find help in your community. You can search for help with food, housing, transportation and more at reduced or no cost to you.
Dental, Hearing and Vision
Make sure your sight, teeth, speech and hearing are at their best. Benefits include:
- Vision. Routine exams and help paying for eyeglasses and contact lenses.
- Dental. We cover cleanings, checkups and dental work.
- Hearing. Tests, check-ups and hearing aids are covered (some limitations apply)
