UnitedHealthcare Community Plan Long-Term Care (LTC) is a coordinated care management program for individuals who are eligible for the Statewide Medicaid Managed Care Plan Long Term Care (SMMC LTC) Waiver Program. Our mission is to keep individuals living independently in their own homes for as long as possible.

This plan is available in the following counties:
Alachua, Baker, Bradford, Citrus, Clay, Columbia, Dixie, Duval, Flagler, Gilchrist, Hamilton, Hardee, Hernando, Highlands, Hillsborough, Lafayette, Lake, Levy, Manatee, Marion, Miami-Dade, Monroe, Nassau, Polk, Putnam, St. Johns, Sumter, Suwannee, Union, and Volusia.
The UnitedHealthcare Community Plan Long-Term Care (LTC) SMMC LTC Program is available in the following counties:
- Region B: Alachua, Baker, Bradford, Citrus, Clay, Columbia, Dixie, Duval, Flagler, Gilchrist, Hamilton, Hernando, Lafayette, Lake, Levy, Marion, Nassau, Putnam, St. Johns, Sumter, Suwannee, Union and Volusia Counties

Benefits & features
We offer a wide range of home, community and personal care services. If you live in a nursing home, an assisted living facility, or an adult family care home, there may be a room and board or patient responsibility cost. There are no costs or copays for other covered services. Services may include:
- Personal care to help you bathe and dress.
- Homemaking and assistance with chores around your home.
- Home-delivered meals or meal preparation.
- Adult day services for social interaction and wellbeing.
- Personal care incontinent supplies not covered by Medicare.
- Respite Care.
- Assisted Living Services.
- Personal emergency help system.
- Occupational, physical, speech and respiratory therapies.
- Home modifications to support mobility.
- Transportation.
- Management and assistance with taking medications.
- Assessment, education, and guidance to you and your family about your nutritional needs.
- Services to address mental health or substance abuse needs.
